Active Monitoring Supported Self-help, is a six week programme with individual support from a practitioner.
We all face difficult times in our lives, sometimes having more information about what we are experiencing helps us to understand ourselves better. Active Monitoring Supported Self-help allows you to learn strategies to manage different life challenges and feel more like yourself. This service is available in Welsh.
Active Monitoring Supported Self-help pathways:
Your practitioner will arrange a weekly call or online session offering support and guidance in your chosen pathway. They will keep in touch with you offering information, workbooks and strategies for you to try.
Active Monitoring pathways support:
Your Active Monitoring Practitioner will arrange a weekly call or online session to offer support and guidance in your chosen area of support. They will keep in touch with you offering information, workbooks and strategies for you to try.

Accessing the service
Active Monitoring is free and available to adults aged 18+
Active Monitoring for Children is available through our SOC project
You can be referred from your GP, or you can sign up HERE
